Course Description
This course introduces participants to the fundamental principles and processes of incident and accident investigation. Learners will gain an understanding of why incidents occur, how to collect and preserve evidence, identify root causes, and recommend corrective actions to prevent recurrence. The course emphasizes a fact-finding, non-blame approach aligned with good safety management practices.

- Define incidents, accidents, near misses, and unsafe conditions.
- Explain the purpose and importance of incident investigation.
- Identify immediate, underlying, and root causes of incidents.
- Collect relevant information through interviews, observations, and documentation.

Certification
Participants who complete the course and assessment receive an Apexis Certificate in Introduction to Incident [Accident] Investigation.
